Why have an in-home assessment?

An in-home assessment gives you about an hour of quality, one-on-one time with a licensed and board-certified Nurse Practitioner, Physician’s Assistant or Physician – right in the comfort and privacy of your own home. Peak providers are specially trained to identify any health issues we can help you with. Even if you’re healthy, this visit is a good opportunity to talk about your health and answer any questions you may have. This assessment doesn’t replace your regular doctor appointments, but is an additional benefit that your health plan offers to you free of charge. While this benefit is completely voluntary, we encourage you to take advantage of all your health plan has to offer.

Take a current health history and ask questions about your personal, social and family history that could affect your health

Review your current prescriptions, including any over-the-counter medications and supplements you’re taking

Perform free health screenings

Answer questions about health issues that are important to you

Give you helpful information about your current health condition and recommend steps you can take to maintain or improve your health

Share the information (with your permission) with your primary care physician so that he/she can better coordinate your care
