What happens during the visit:

During a prospective health assessment, Peak providers will take a current health history, review all medications, perform labs and tests as required by the plan and recommend follow-up care. They can also leave behind plan materials to ensure they make it directly into the member’s hands.

What happens after the visit:

Peak notifies PCP’s and/or the plan of all urgent findings immediately; referrals and recommendation reports are sent as required by the plan. Our clinical team completes QA and record coding after each visit to capture any compliance gaps. Additional follow-up can be accomplished through member surveys.
