Peak can help improve your overall patient care
Peak’s goal is to help your plan promote a higher quality of life and provide exceptional care for your Medicare Advantage, Medicaid or Commercial Health Plan members. Our licensed and credentialed Nurse Practitioners, Physician’s Assistants and Physicians perform prospective health assessments by evaluating the current health conditions and home environment (for in-home assessments) of your members and determine what changes might benefit both them and your plan. With our unique targeting methodology, we are able focus on members who are disconnected from the delivery system, those with significant gaps in quality metrics and those with persistent HCC conditions, ensuring that those members who will benefit most from a personalized visit are seen first. What happens during the visit:
During a prospective health assessment, Peak providers will take a current health history, review all medications, perform labs and tests as required by the plan and recommend follow-up care. They can also leave behind plan materials to ensure they make it directly into the member’s hands.
What happens after the visit:
Peak notifies PCP’s and/or the plan of all urgent findings immediately; referrals and recommendation reports are sent as required by the plan. Our clinical team completes QA and record coding after each visit to capture any compliance gaps. Additional follow-up can be accomplished through member surveys.
